Notice of Privacy Practices

Artistic Smiles Dentistry — Philip Nguyen, DDS (“Dr. Phil”) · Effective July 28, 2026

We are committed to protecting the privacy of your health information. We are required by law to maintain the privacy of your protected health information (PHI), to provide you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

How we may use and disclose your health information

Treatment. To provide and coordinate your dental care — for example, sharing information with specialists or dental labs involved in your treatment.

Payment. To bill and obtain payment — for example, submitting claims to your insurance.

Health care operations. To run our practice — quality review, staff training, scheduling, and business management.

Appointment reminders & related communications. We may contact you by phone, text, or email to remind you of appointments, confirm requests you submit through our website, or tell you about treatment options or services that may benefit you.

Business associates. We may share information with vendors who perform services for us (scheduling, patient communication, records hosting, billing). We require them by written agreement to safeguard your information.

Other uses and disclosures permitted or required by law

We may use or disclose your information without your authorization in certain situations, including: as required by law; public health activities; reporting abuse, neglect, or domestic violence; health oversight; lawsuits and disputes; law enforcement; coroners, medical examiners, and funeral directors; organ/tissue donation; approved research; to avert a serious threat to health or safety; specialized government functions; workers’ compensation; and correctional institutions.

Uses that require your written authorization

Most uses and disclosures not described above will be made only with your written authorization — including marketing that involves payment from a third party and any sale of your health information. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

Your rights

• Access — inspect and copy your health and billing records.
• Amendment — request correction of information you believe is incorrect or incomplete.
• Accounting of disclosures — request a list of certain disclosures we made.
• Restrictions — request limits on how we use or disclose information; we must agree to restrict disclosure to a health plan for items you paid for in full out of pocket.
• Confidential communications — ask us to contact you a certain way or place.
• Paper copy — obtain a paper copy of this Notice at any time.
• Breach notification — be notified if a breach affects your unsecured health information.

To exercise these rights, contact our Privacy Officer below. Some requests must be in writing.

Our responsibilities

We are required to maintain the privacy of your health information, provide this Notice, follow its terms, and notify you if a breach occurs that may have compromised your information.

Changes to this Notice

We may change this Notice and make the revised Notice effective for information we already have as well as information we receive in the future. The current Notice is posted in our office and on this website with its effective date.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights (www.hhs.gov/ocr/privacy/hipaa/complaints). We will not retaliate against you for filing a complaint.
